Program Objectives
The Care to Confidence Program is designed to bridge the gap between paediatric care and adult health systems to ensure young people do not fall through the cracks during this vulnerable stage of transition.
Transition Roadmap & Training:
1Teach skills like communicating with healthcare providers, understanding prescriptions, and on managing T1D in university, NYSC, and early career settings, covering topics like meal planning on a budget, managing glucose during long workdays, and navigating health insurance.
